Health Care Flexible Spending Account (FSA) Overview
An FSA allows employees to use pre-tax dollars for eligible medical, dental, vision, and health-related expenses, helping reduce out-of-pocket costs.
Contributions
Employees can contribute up to $3,300 in 2025 to a Health FSA through payroll deductions, as per IRS guidelines. However, employers may choose to set a lower contribution limit for employees. Employers may also contribute to the Health FSA, but they are not required to do so. Health FSA funds are made available in full at the start of the plan year, regardless of the employee's current contribution balance.
Coverage
Eligible expenses include:
- Medical: Doctor visits, co-pays, deductible, prescriptions
- Dental/Vision: Deductibles, Exams, glasses, orthodontics
- Medical Equipment: Devices, first-aid supplies
Non eligible expenses include:
- Toiletries, cosmetics, and lotions
- Cosmetic procedures
- Canceled or missed appointment fees
- Insurance premiums
Visit sigis.org for a list of merchants using the Inventory Information Approval System (IIAS). Card purchases made at these merchants will be automatically approved for
Health FSA eligible items. Certain expenses may require a prescription or doctor’s note (letter of medical necessity). Check coverage in the Level app.
Eligibility
Enrollment typically occurs during open enrollment, at hire, or after qualifying life events. Check the Level app or contact HR for details.
Making a Purchase
You can use your Level Visa® card or submit a claim in the Level app for reimbursement.
- Card Approval: If the merchant doesn’t use an Inventory Information Approval System (IIAS) to automatically identify eligible medical expenses, your Level Visa card may be declined. For a higher chance of card approval without needing a receipt, consider purchasing eligible Health FSA items at the pharmacy counter instead of a general checkout register.
- If Your Card is Declined: Use another form of payment, then submit a reimbursement claim in the Level app with an itemized receipt and necessary documentation to receive reimbursement.
- If Your Card is Approved: You may still be required to verify your transaction by submitting a copy of your itemized receipt. You’ll be prompted to upload a receipt in the app immediately after your purchase.
Grace Period
Unused funds may be forfeited at year-end, but some plans allow a grace or run-out period. Some FSAs offer a 2.5-month grace period (through March 15th) to use remaining funds after year-end. Your plan may also allow you to carry over funds between benefit years. View your plan documents in the Level app to learn more.
Termination
When you leave the company, remaining funds are managed per company policy and IRS rules. COBRA may allow FSA continuation. Review termination rules in the Level app or contact HR.
