Family Planning Plan overview
Level offers two Family Planning Plans for financial support related to family planning: the Family Planning Health Reimbursement Arrangement (HRA) and the Family Planning Lifestyle Spending Account (LSA).
Coverage
- Family Planning HRA: Covers adoption costs, surrogacy expenses, and medically necessary fertility costs
- Family Planning LSA: Covers adoption and surrogacy costs and elective fertility preservation (like egg/sperm freezing), but excludes medically necessary fertility preservation.

Eligibility
- HRA: Restricted to employees with HMO/PPO plans
- LSA: Available to all employees

Termination
If you leave the company, remaining benefits are managed according to your employer’s policies, so check with HR for specifics.
