Dental Plan overview
Level’s Dental Plans support oral health by covering a range of dental services. Providers generally submit claims directly to Level, but if upfront payment is required, use a personal payment method and submit a statement in the Level app.
Coverage
The Dental Plan covers:
- Preventive care: Exams, cleanings, X-rays
- Basic restorative services: Fillings, root canals, extractions
- Major restorative services: Crowns, bridges, dentures
- Orthodontics: May include braces or aligners, depending on your plan

Eligibility
Enrollment typically occurs during open enrollment, at hire, or after qualifying life events. Eligibility is employer-determined, with many plans extending to spouses, partners, and dependents under 26. Check the Level app or contact HR for details.
Cost estimates
In the Level App, view cost estimates by navigating to Accounts > Plans > Dental and selecting treatments under Coverage highlights. You can compare in-network and out-of-network prices.
Finding a provider
Search for providers by name, specialty, or location in the app. In-network providers display a check mark and offer discounted rates.
Level Network
Level benefits can be used with any U.S. provider, with in-network providers offering discounts.
Submitting receipts
If you pay out of pocket, submit a claim with an itemized receipt in the Level App within 180 days of service. Receipts must include date of service, patient and provider names, services, and payment details.

Termination
If you leave the company, remaining benefits are managed according to your employer’s policies, so check with HR for specifics.
