Family Planning Plan overview
Level offers two Family Planning Plans designed to provide financial support for family planning needs: the Family Planning Health Reimbursement Arrangement (HRA) and the Family Planning Lifestyle Spending Account (LSA).
Coverage
The Family Planning Plans include the following coverage details:
- Family Planning Health Reimbursement Arrangement (HRA): This plan covers various costs associated with family planning, including:
- Adoption costs
- Surrogacy expenses
- Fertility costs
- Family Planning Lifestyle Spending Account (LSA): This account provides coverage for:
- Adoption and surrogacy costs
- Elective fertility preservation, such as egg or sperm freezing
- Note: Elective fertility preservation is not covered when it is considered medically necessary
Eligibility criteria
Eligibility for the Family Planning Plans is determined by employer policy. For the Family Planning HRA, health plan enrollment may be required.
Eligibility
-
HRA: Restricted to employees with HMO/PPO plans
-
LSA: Available to all employees
Check specific criteria in the Level App or consult HR.
Auto-enrollment
Automatically enrolls members based on plan enrollment criteria (e.g. employee type, status, location) without a required election by the employee.
This enrollment type is most popular for Family Planning Accounts, but check out our Enrollment article for more options.
Termination
Upon an employee's departure from the company, the treatment of any remaining benefits related to family planning will be based on the employer's policy. The specific treatment may vary, so it’s essential to confirm the company’s stance and communicate it to departing employees if needed. All terminated employees will continue to have access to their Level account only to review past transactions and expenses.
