Level provides PPO dental plans designed to simplify patient care and enhance the provider experience. Level’s dental plan offers a straightforward PPO option with no waiting periods, no missing tooth clause, Enjoy hassle-free claims with no downgrades on materials for fillings or crowns, and no frequency limitations.
Level ID Card
Patients with Level PPO coverage can access their virtual ID card, featuring a unique 10-digit subscriber ID, directly through the Level app. Each dependent on the plan also receives their own distinct subscriber ID.
Dental Plan Overview
All Level plans are PPO plans. Most of our plans provide full (100%) coverage for all eligible dental services, up to the annual maximum, while other plans have coinsurance associated with basic, major and orthodontic procedures. Visit the Plans tab on the Provider Dashboard to secure code coverage.
Preparing for a Level Member Visit
Level plans include no deductible, no waiting period, no missing tooth clause, no downgrades, no group numbers, and no frequency limitations. Verify patient eligibility prior to appointments by logging into the free Provider Dashboard with the subscriber ID or last 4 SSN, DOB, and last name. Sign up or log in here.
Submitting claims made easy using Provider Dashboard
The easiest and fastest way to submit claims is through the Provider Dashboard. Using the Provider Dashboard is the quickest route to getting your claims paid as most claims submitted this way are processed the same day! Once you searched your patient's account, click submit claim and follow the claim prompts.
Additional claim submission options are:
- Electronic Claim Submission: submit claims electronically through your practice management system using payer ID LEVEL
- Email: If you are unable to submit your claim via Provider Dashboard or electronically, please email a claim to support@level.com.
- Mail: If you find it necessary to mail in a claim, please send it to: LEVEL- Claims Dept - PO BOX 176, NY NY 10013. Please be aware that mailed claims might be subject to delays in processing. To avoid any potential holdups and to expedite your claim, we strongly recommend utilizing our electronic submission options. Thank you for your cooperation!
