Level ID
Each member has a virtual dental ID card with a unique 10-digit subscriber ID, accessible in their member app. Each dependent covered under the plan will also have their unique subscriber ID. Members can access this information through their member app and readily provide it to you upon your request.To obtain a dependent’s Level ID:
Ask the member to toggle to the dependent’s profile in the Level App.
Alternatively, use the Family section in the Provider Dashboard to view their ID and effective dates.
