Your claims

File a claim

After your visit, your dentist should file a claim for you. We’ll send you a notification when we receive your claim, and another notification when the details are ready.

If your dentist doesn’t file a claim for you, you can file your own claim and get reimbursed. Tap Help then File claim in the Level app. You’ll provide the dentist’s name and phone number, the cost of the visit, and the date of the visit. 

View a claim

When you open the app, you’ll see a list of your recent dentist visits. Select one of your claims to see a detailed breakdown of your visit, including procedures, cost, and how much you owe. If you want more details about the visit, tap See details at the bottom of the page.

If you’d like a paper copy of your claim, send us a note or call us at (855) 400-5705.

Dispute a claim

If a charge or treatment on a processed claim looks incorrect, contact us from the app or call us at (855) 400-5705.
