Understand your plan

Network

Level contracts with dentists to create a network that gives members flexibility and savings. When you search for dentists in the app, you’ll see who’s in and out of the network. You can always visit an out-of-network dentist, and get reimbursed, but we can’t guarantee how much they’ll charge and what will be covered by your plan.

Benefits

Head to the Benefits page to see what your plan covers (your plan may not cover the entire cost of every type of treatment). 

Say your plan covers 100% for teeth cleanings, and your current benefits balance is $200. If you get a cleaning for $150, your plan will cover the entire visit, and your new benefits balance will be $50. 

If you get a cleaning for $250, your plan will cover $200 for the visit, and your new benefits balance will be $0. You’ll owe the dentist $50, which you can pay right from the app.

Plan period

Your coverage begins on the first day of the month after enrolling in Level, and ends on the last day of the month you leave your company—unless you opt for continued coverage through COBRA. Contact your company’s benefits representative to learn more about COBRA. 

Dependents

You can add dependents to your account during open enrollment or in a qualifying life event. If your plan period has already started and you need to add a dependent, contact us through the app or call us at (855) 400-5705.
