Remaining balances and recent activity

The Level app helps you keep track of your remaining benefits balance. To view your balance, log into the Level app, and view the balance on the Activity screen. Your benefit balance will be displayed. Tap on a balance to see more details about your remaining balance and past activity. Your visit information is displayed on the Activity page of the app.

Network

Level members can use their benefits at any provider, whether they are in or out of network. In-network providers have a checkmark next to their name. This means this provider offers lower rates to Level members, so your cost may be less than going out of network. Out-of-network providers may charge higher rates for their services. To confirm rates for out-of-network providers, get in touch with Level Support by calling (855) 400-5705 or emailing support@level.com.

Dental benefits

Your Level dental plan covers dental care up to your plan’s annual maximum.  You can download your benefits booklet in the app. It’s found in the three-dot menu.  You can also reach out to Level Support at (855) 400-5705 or support@level.com for assistance.

For example, if your plan offers 100% coverage for routine cleanings and you walk into your dental visit with a benefits balance of $200, the total of your cleaning will be deducted from your current benefits balance. If the treatment cost for the visit is above your benefits balance, you are responsible for the difference.

Vision benefits

Level’s flexible vision plan covers vision care up to your plan’s annual maximum. Eye exams, eyeglasses, and contacts are covered. With no frequency limitations, you are able to use your vision benefits on all eligible vision services and products until the annual max is exhausted.
