You can check dental eligibility in real time through Level’s provider dashboard. The provider dashboard offers a comprehensive view of members’ dental plans. This includes coverage details, treatment history, maximum allowances, accumulators, and deductibles.
If you have not yet registered for Level’s provider dashboard, eligibility verification details are available over the phone or by email from Level support Mon-Fri, 9 am-9 pm ET at (855) 400-5705 or firstname.lastname@example.org.
Level members may pay for services with the Level Card. The card is authorized to cover any eligible charges, up to an annual max. If the member has a physical Level Card, run the card at your point-of-sale system like any other payment card. If the member has a virtual Level Card, visible in the Level app, manually enter the card details into your point-of-sale system.
Some members won’t have a Level Card. In these cases, submit a claim for payment detailing the record of services through the provider dashboard, electronically through your practice management (PM) system, or sending an ADA claim form via fax, or by mail.
Our payer ID for claims and NEA attachments is LEVEL. For SoftDent users only, the claims payer ID is 01206.
In cases where the Level member pays out of pocket, they will need to submit a reimbursement request using the Level app. Once Level confirms the details of the visit and processes the request, the member will be reimbursed.
