File a claim
Using the provider dashboard
The quickest way to submit and receive payment for claims is by using the provider dashboard.
To get started, sign up for an account. It's free to sign up, and you'll find that many claims submitted through the dashboard are processed and paid the same day.
Using your practice management system
To send claims through your office’s practice management system, simply refresh your system and add Level as a new insurance company. Our payer ID for claims and NEA attachments is LEVEL. For SoftDent users only, the payer ID is 01206.

Note: Electronic submissions using the provider dashboard or your practice management system are received and processed faster than claims sent by postal mail or fax.

View claim status
You can view the status of a claim through Level's provider dashboard.
If you are not registered for Level’s provider dashboard and have questions about a processed or pending claim, contact our support team at support@level.com or at (855) 400-5705. You'll be asked to provide the patient's Level ID or social security number, the name of the patient, their date of birth, and the date of service of the claim in question.
Get paid
Once a claim is processed, we'll send a check to the billing address listed on the claim. If a member pays for dental or vision using their Level Card, you’re paid immediately.
Level will send an Explanation of Payment (EOP), which will provide a summary of treatment, total charges, a detailed overview of Level’s payment responsibility, and a detailed overview of any member responsibility, if applicable. A similar document will be sent to the member to notify them of their responsibility, if any.
Appeal a claim
For claim appeals or corrections, contact Level’s support team at (855) 400-5705 or support@level.com. Be prepared to provide the date of service, the amount paid in office by the patient, and the amount charged.
