At a dental or vision provider

If you have a Level Card to pay for services, bring it to your appointment so you can pay at your provider’s office. If you have a physical Level Card, be sure to activate it by following the prompts in the Card section of the Level App.

At your visit, share your Level ID. This is a 10-character mix of numbers and letters, beginning with "LVL." It’s printed on your virtual card. To find your virtual card, visit the Card section of the Level App. 

If your provider isn’t familiar with Level at the time of your visit, they can reach out to Level Support at (855) 400-5705 to learn more about your coverage. 

If you have the virtual Level Card, your card information can be entered into any point-of-sale system. If you have the physical Level Card, just use it like any regular payment card.
