Check claim status

You can view the status of your claims in the Claims section of the Provider Dashboard. If you don't have a dashboard account and would like to know the status of your claim, contact Level Support at (855) 400-5705 or You’ll be asked to provide your tax identification number (TIN), the treating provider’s name, and your office address, as well as the patient’s name, date of birth, and Level ID or the last four digits of their social security number.
