Appeals

To appeal a claim decision, contact Level Support at (855) 400-5705 or support@level.com.  You’ll be asked to provide the date of service; your tax identification number (TIN); the treating provider’s name and office address; and the patient’s name, date of birth, and Level ID or the last four digits of their social security number. You should include the reason for the claim appeal, as well as any clinical documents needed to support your request. Level may ask for additional supporting documentation to review your claim for reconsideration.
