While Level’s vision plans are generous, they don’t cover everything. The following treatments are generally excluded from coverage: 

  • Vision membership plans or subscriptions
  • Investigational or experimental procedures 
  • Most medical procedures

We cover Level 1–4 medical exams, including glaucoma screening, extended retinal exams, and diabetic exams. Dry eye specialty testing and foreign body removal are also covered. 

Non-prescriptive lenses and frames, like blue-light glasses, are covered only if accompanied by a note from a provider describing a specific medical condition. 


Why did I get a call from Level’s support team? 

Members can request that Level reach out to their providers in advance of an appointment, to make sure everything runs smoothly. At a member’s request, our support team will call your office to introduce Level, explain the member’s benefits, and answer any questions about their plan.
