At your visit, share the Level ID printed on your virtual card in the app so your provider can submit a claim to Level. If your provider isn’t familiar with Level at the time of your visit, they can reach out to Level Support at (855) 400-5705 to learn more about your coverage. 

If your provider requires payment up front, use your Level Card. If you use your Level Card to pay for care, be sure to get an itemized receipt or statement of services, then add it to your transaction in the Level App.
