Before your first dental visit
Provider guide
The Level Provider Guide gives your dental provider the information they need to work with Level. Download and share it with them before your appointment:
- Sign in to your Level account
- In the Activity section, select your dental plan
- Go to the Details tab
- Select Documents
- Download the Provider Guide
If they need to, your provider can set up a free online provider dashboard, email support@level.com, or contact support at (855) 400-5705 to learn more about your coverage before your visit.
Level ID
Your Level ID is a 10-character code starting with "LVL" and can be found on your virtual card in the Card section of the app. Share your Level ID at your appointment so your provider can submit claims directly to us.
Submitting a claim
Providers should submit claims directly to Level. Even if your dentist says they don’t accept Level, all dental providers can file a claim with us, regardless of network status. If needed, have your dentist call (855) 400-5705 or visit our Provider Help Center more details.
If your provider requires upfront payment, use your personal payment method and ask for an itemized receipt or statement of services that includes:
- Date of service
- Patient’s name
- Provider’s name
- Services rendered
- Amount paid
You can submit a claim for reimbursement with this receipt as long as it's within 180 days of the service date.
